An administrator connects your Microsoft 365 tenant once. From there you choose which security groups become directory entries, or turn on whole-directory sync and take everyone. Nothing commits until you have seen what it would do.
Directory contacts that live in Entra or Active Directory without a mailbox or a licence come across too. That is usually where the referring physicians, the funeral homes and the DME reps have been sitting all along, invisible to anything that only reads mailboxes.

The sync only modifies records it created. The pharmacy number a coordinator added by hand last winter stays exactly as she wrote it, and matching is done on each person’s permanent directory identifier rather than their email address, so a name change or a new address does not create a duplicate.
A nightly sync keeps the directory in step with Microsoft, and there is an Import now button for the day somebody joins and you need them in the on-call sheet this afternoon.
Beyond contacts, staff can sign in to Care Roster with their Microsoft account. Access then follows your directory instead of a separate list somebody has to maintain.
Map an Entra security group to the Administrator, Editor or Viewer role, and people land in the right role the first time they sign in. Move somebody between groups and their access moves with them.
When an account is disabled in Entra, that is checked at the moment the person tries to sign in, so access ends with the account rather than at the next nightly run. You can also have people removed from a mapped group deactivated automatically.
Email and password sign-in keeps working alongside it, and everything in Care Roster works without Microsoft 365 at all. This is something you switch on if it helps, not a dependency you take on.

No. The sync is one way and read only. Care Roster reads your directory through Microsoft Graph and never writes to it, so nothing it does can alter a record on the Microsoft side.
